PROCEDURE

 

 

Step 1:

An individual file a dossier to Drug Administration - Ministry of Health;

Step 2:

Drug Administration shall receive and examine the dossier;

Step 3:

Within 30 working days from the date of receipt of a valid dossier, if there is no additional request, the Drug Administration shall organize an appraisal to issue a pharmacy practice certificate to the applicant. If not, the Drug Administration shall reply in writing and clearly state the reason;

Step 4:

Return the result to that applicant.

SUBMISSION OF DOSSIER

Submit the dossier by post or directly submit at the Drug Administration.

DOSSIER

01 dossier.

Elements:

  1. A proposal form of issuing pharmacy practice certificate;

  1. 02 photos size 4x6 of the proponent within 06 months;

  1. Valid copies of qualifications suitable for professions;

  1. A certificate of health suitable for providing healthcare services issued by an eligible healthcare facility or a certificate of health issued by a foreign healthcare facility within 12 months from the date of dossier submission;

  1. A certificate of internship issued by the head of the facility that proponent practiced;

  1. Certificate of completion of training program, updating professional knowledge in pharmacy;

  1. Certified copy of citizen identity card, identity card or passport;

  1. A criminal record.

DURATION

30 days from the date of receiving a complete dossier.

SUBJECT

RESULT

Valid dossier: Pharmacy practice certificate;

Invalid dossier: Notice of dossier completion.

  • Decree no. 54/2017/ND-CP guidelines for implementation of law on pharmacy;

  • Circular no. 41/2011/TT-BYT guiding the grant of practicing certificates to medical examination and treatment practitioners and operation licenses to medical examination and treatment facilities.
