PROCEDURE | |
| Step 1: | An individual file a dossier to Drug Administration - Ministry of Health; |
Step 2: | Drug Administration shall receive and examine the dossier; |
Step 3: | Within 30 working days from the date of receipt of a valid dossier, if there is no additional request, the Drug Administration shall organize an appraisal to issue a pharmacy practice certificate to the applicant. If not, the Drug Administration shall reply in writing and clearly state the reason; |
Step 4: | Return the result to that applicant. |
SUBMISSION OF DOSSIER | Submit the dossier by post or directly submit at the Drug Administration. |

Elements: | A proposal form of issuing pharmacy practice certificate;

02 photos size 4x6 of the proponent within 06 months;

Valid copies of qualifications suitable for professions;
|
A certificate of health suitable for providing healthcare services issued by an eligible healthcare facility or a certificate of health issued by a foreign healthcare facility within 12 months from the date of dossier submission;
|
A certificate of internship issued by the head of the facility that proponent practiced;
|
Certificate of completion of training program, updating professional knowledge in pharmacy;
|
Certified copy of citizen identity card, identity card or passport;
|
A criminal record.
